The Quality of Life Expo will be held in Navy Pier’s Lakeview Terrace and will offer participants a chance to learn about and engage in services, products and other opportunities that will enable them to continue on with their active lifestyles.

**Electricity is not included. Any additional equipment or set-up requirements must be arranged at the exhibitor’s expense directly with the exhibition company, including freight shipment for any materials..
GOODY BAG INSERTS
$450 - Shipping deadlines and shipping address will be sent out after payment
has been received.
4,500 inserts, whether it be a coupon, sample, or other like offering, will be included in all participant goody bags which they will receive at packet pick up at the event.

A non-refundable deposit of 50% of the total booth fee reserves each booth. Final payment is due October 22, 2012. No booth space will be confirmed until such time as the deposit is received.
CANCELLATION
An exhibitor may not cancel or withdraw their application once it has been confirmed by Chicago’s Perfect 10. We do not offer refunds to vendors who choose not to attend.
